jotform please help

As you can see from the screenshots, I have gone into 'form styles' to remove the highlighted heading, yet the highlighted heading is not disappearing

also I have made 'name' the top input' and 'email required' the 2nd input.. but on the live form it hasn't changed..

My changes are not flowing through to the form. I have refreshed the browser, installed flock browser which I never use, just to test it.. and I deleted all cache and cookies. help
